2019 Madison County Idaho Community Preparedness Survey

Instructions

To Whom It May Concern:

Madison County is conducting a study to better understand the preparedness needs and risk perceptions of its residents as part of the County’s Hazard Mitigation Plan update process. To do so, a questionnaire has been distributed throughout the County, and you have been selected to participate. Your feedback is greatly needed and appreciated!
 
The questionnaire should only take about 10 minutes to complete. All responses will be kept confidential, and your participation is strictly voluntary. Your input will enable the County to better serve you.
 
Prize
As a thank you for completing the survey, you have the option at the end of the survey to enter into a prize raffle. The Contractor hired by Madison County to complete the Madison County Hazard Mitigation Plan is providing the prizes. Prizes include weather radios, emergency preparedness kits, and more.

DEFINITIONS

Hazard Mitigation:  The purpose of hazard mitigation planning is to identify policies and actions that can be implemented over the long term to reduce risk and future losses. Mitigation forms the foundation for a community's long-term strategy to reduce disaster losses and break the cycle of disaster damage, reconstruction, and repeated damage.
